• Proof of address: The Gas Company, Riverside Public Utilities, or Edison utility statement is acceptable. (Original statement portion of the most current bill, dated within the last 30 days, with the top name listed in parent/guardian name.)

• At least two contact names with phone numbers.

If your student will not be returning to Stokoe Elementary School for the new school year, please complete the following and return to our school office:
