REGISTRATION

KINDERGARTEN

Important Dates: MARCH 2007

Children must be 5 years of age on or before December 2, 2007.

Registration forms will be available in the office beginning: March 12-16, 2007

Registration hours: 9:00-11:00 a.m. and 1:00-3:00 p.m. by appointment, appointments will take priority over walk-ins.

Please bring the following required items (we will need original documents, no photocopies):

1. Proof of Address

The Gas Company, City of Riverside, or Edison utility bill. Must be detailed portion of the bill in one of t he parent’s names, and current within the last 30 days.
Current social service eligibility papers, i.e. AFDC, GAIN, CAL WORKS, SSI, Unemployment.

2. Certified Birth Certificate from the County Registrar.

3. Completed Immunization Records - recorded by the doctor’s office.

4. State of California, Report of Health Examination for School Entry, (Exam date must be 1/1/07 or after).

5. At least two contact names with phone numbers.

6. Language testing for bilingual or non-English speaking students.

All incoming kindergarteners will be assessed before being assigned to a class. You will schedule assessment time after registration has been completed.

We are unable to accept incomplete registrations.

In order to guarantee placement on the same track as currently enrolled siblings, registration must be completed as early as possible.

PRE REGISTRATION FOR RETURNING STUDENTS FOR 05-06

Pre -registration for returning Myra Linn students will begin May 2007. Registration hours are 9:00-11:00 A.M and 12:30-3:00 P.M. You will receive the registration packet on April 2007 (R/O/G) and May 2007 (B). It is important that you provide proof of residency in order to pre-register. Please have all documents completed before coming in to pre-register. ALL STUDENTS MUST PRE REGISTER IN ORDER TO BE PLACED ON A CLASS LIST FOR THE 07-08 SCHOOL YEAR.

EVENING REGISTRATION FOR RETURNING STUDENTS

Two evening registrations will be scheduled for your convenience. The dates will be announced for May and June 2007 from 5:00 P.M. to 7:00 P.M.
